Permission and Consent

Indemnity Waiver and Release of Liability.


This agreement is made between the above parent/guardians and the teachers at Waves and Woodlands. Waves and Woodlands and their location partners agree to provide educational services for the following child/children listed in this form.



The Parent(s)Guardian(s) acknowledge and agree that:

  • Outdoor Educational services inherently involve certain risks, including but not limited to accidents, injuries, or illnesses.

  • To the fullest extent permitted by law, the parent(s)guardian(s) hereby release, waive and discharge Waves and Woodlands and their partners from an all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness that may be be sustained by the child(ren) while under the care of the school.

  • Indemnification: The parent(s)(guardian(s) agree to indemnify and hold harmless Waves and Woodlands from any and all claims, liabilities, damages, costs, or expenses (including reasonable attorney fees) arising from:

    1. Any claims brought by or on behalf of the child(ren).

    2. Any claims arising from instructions, information, or materials provided by the parent(s)guardian(s).

    3. Any injuries, damages, or losses not caused by gross negligence or willful misconduct.

  • Medical Authorization: The Staff at Waves and Woodlands is First Aid and CPR trained and are authorized to seek emergency medical treatment for the child(ren) if necessary and if unable to reach the Parent(s)(Guardian(s). The Parent(s)Guardian(s) agree to be financially responsible for any medical treatment provided.

  • Entire Agreement: This waiver and release constitute the entire agreement between the parties and supersede any prior discussions or agreements, whether written or oral.

I give my child permission to participate in all outdoor forest school activities, including walking off site with staff supervision.
Emergency Medical Treatment: In the event of an emergency, I authorize the Waves and Woodlands Forest School staff to seek medical treatment for my child.
Photographs/Media: I give my permission for photos/videos of my child to be used for:
